Clarification failure

I asked my nurse what forms I am on.

He went and checked and said I am on a Form 4.

I asked if I am on a Form 33.

He said no, there is only a Form 4 on the board.

I asked him to double check and get back to me when he was able.

He told me not to hover near the nursing station, that the request was not a priority, and that he would get back to me when he was able.

Staff should know exactly what Forms I am on, and when they expire.
