Notice of Appeal (Served + E-Filed via E-mail to SCJ + All parties) for CCB's 25-4717-02

Here is my Notice of Appeal for the decision rendered by the Ontario Consent and Capacity Board (cc'd) 25-4717-02 which was received on October 31, 2025.

As I have had numerous issues filing online (via JSO), I am using this alternative method of filing via E-mail. As with my appeal of the SCJ's decision in CV-25-99970 to the Court of Appeals, I am CCing all parties (and their corresponding legal counsel) to meet the criterion of proof of service, since I can not afford a process server as I am on ODSP (attached is the email from staff at the Ontario Court of Appeals).

Should this be deemed an unacceptable means of serving and filing, given the URGENT circumstances (I have 7 days according to the HCCA), please notify me immediately and provide me and all included parties with the means whereby I should serve and file this notice of appeal by responding to this e-mail.

All recipients of this email: if there are any errors within the notice of appeal, please notify me immediately and please also confirm receipt of the notice of appeal as soon as possible.

I also hereby formally request an extension in time for filing the notice of appeal, for filing a supplementary notice of appeal, and for filing the appellant's factum.
